Before the incident
1. TM was trained on how to properly use the table saw to cut stiles inserts. 2. He began cutting the wood using the proper technique of using push stick. 3. Stopped using push stick to slide blocks through the guard into the blade. 4. Pushed block with thumb under the guard and cut thumb on the blade.
What happened
The TM was cutting wood stile inserts using a table saw. He was just trained on how to properly use it and he was given the okay to continue to cut. He cut the first 5 pieces correctly using push stick the next 3 he stopped using the push stick and thumb went under the guard coming in contact with the blade causing small laceration to thumb that required stitches.
