Before the incident
1. The TM was taking quality measurements of the panel. 2. The line switched from 18 foot sections to 9 foot sections closing the shear. 3. The shear pinned him between the press. 4. The 2nd operator was able to rehome the line and free the operator.
What happened
The TM entered a guarded (restricted) area to take a quality measurement. When he entered into the area to make the measurement the run switched from 18 foot to 9 foot causing the shear to slide forward towards the press and pinned him against the press. The other machine operator was able to release the shear freeing him. This resulted in 4 cracked ribs. The TM was taking a short cut quality measurements are taken outside of the guarded area 7 feet from where he entered.
