Associate was retrieving a part 's dunnage from a double-stacked tote to use in packaging work.

Associate lowered a side access door (of the double-stacked tote) to obtain and remove dunnage. After obtaining and removing dunnage associate pushed the door access panel up to close it but it did not latch. The side door access panel swung back open down striking associate in the head.

Associate remained at work without any further issues other then minor tenderness. 48-hours later associate then decided to go to ER for evaluation. ED physician diagnosed associate with mild concussion and had associate stay off of work for the remainder of that day (day of evaluation). Return to work written for next day.
