Team member was driving a stand up forklift.

Team member stated while driving a stand up forklift she was putting away a pallet on isle [REDACTED]-F01in picking that the rack collapsed. There was a pallet on the rack that contained bottles of soap. The soap bottle began to fall hitting her on her left wrist and lower back. [REDACTED] [REDACTED] and Radial 's Supervisor [REDACTED] asked would she like to seek medical attention but she refused. On [REDACTED] she stated that she was having pain and her left wrist and back and would like to visit a doctor.
