Before the incident
TM was called to check a box of tiles that were stuck and not moving on the line. The belt had came off the guide.
What happened
At approximately [REDACTED] a Maintenance Technician attempted to slide on the belt on the packer conveyor that was twisted. The CPK machine was still on while the Technician attempted to slide the twisted belt onto the guide. CPK remained in its cycle and thus causing severe amputation to the left-hand fingers.
