At [REDACTED] the employee approached the forklift while was moving forward to move some boxes. The employee did not warn the driver that he was going to take something the back of the forklift chair so the driver did not dee him and continue moving and hit the employee with the forklift in his leg and foot who made him fall in to the ground and hit his right arm and back of his head. The employee was sent to Woodwinds hospital to be checked.
