Before the incident
The employee was rotating positions with coworkers at the grader station and was heading to the east side of the grader.
What happened
While ascending the grader stairs to the catwalk she tripped on the toe board of the catwalk causing her to fall on her knees and reach out with her left arm to grab the mid-railing.
Injury or illness
Left Shoulder Pain. Update entered on 11 13 2024: Per the orthopedists instructions on 11 01 the employee was referred to a surgeon for further evaluation of the injury. At her appointment on 11 12 2024 the Dr. informed the employee that the diagnosis was a left humerus surgical neck fracture and referred her to physical therapy. Restrictions on her left arm remained until the next follow-up appointment on 01 02 2025.

The sudden reach to grab the catwalk handle with the left arm. Employee was distracted while going up the stairs due to putting her gloves on and not holding on to the railings.
