Performing routine exhaust fan maintenance removing the fan shroud on unit 45 with gloved hands

While removing the fan shroud on unit 45 the fan was freewheeling and glove got pinched between belt and pully which caused injury to the left thumb.

Improperly lock out tag out the freewheeling fan and pully assembly and use PPE gloves on rotating equipment. Employee retrained on LOTO.
