Before the incident
[REDACTED] was attempting to assist a truck driver in moving a check fixture on a semi-flatbed. The forklift could not access it until it was moved to align it with the drive path.
What happened
One of the wheels dropped into a divet or hole in the flatbed trailer. Since it is very top-heavy the fixture and cart started to flip over the side of the trailer. [REDACTED] instinctively reached for it and it caused him to fall off the side of the flatbed striking his head and getting hit by the fixture.
