What happened
EE was in [REDACTED] under pusher 2 with the operator looking into a problem with table one. EE told the operator to go up top and start the machine so he could see table one 's movement. The operator started up the machine and pusher 2 moved towards EE; EE then tried to move quickly out of the way tripping over a beam & falling backwards onto his outstretched right arm hand causing pain to his right shoulder.
