Before the incident
The employee was setting up the line during a changeover. The task required the employees to kneel on the edge of the machine to make adjustments to vacuum pads. As the team member was stepping down his foot slipped on some oil and water on the floor causing him to fall.

When the team member fell he landed on his backside and his right elbow struck a step on the machine causing a laceration.
