Before the incident
Worker was securing equipment on vessel deck with chains and binders.
What happened
Worker went over to put the chain hook(from the bind bar) in once the bar was pushed down far enough by the other ILA worker. When she was kneeling over to connect he hook another worker came over to help push the lashing bar down for a tighter lash. As the workers pushed down on the bar it slipped off the lashing bar causing the bind bar to come up and come in contact with [REDACTED] 's upper right forehead area causing a small laceration.
