While using plier 's employee had wrong angle or reached too far causing pliers to get pulled into the a gap

Employee got pliers pulled into machines a gap and held onto them instead of letting them go. He attempted to pull them out and twisted tweaked his wrist. No part of the hand was ever pulled into the nip at any point. Injury was a result of him wrestling with pliers in attempt to prevent damage to equipment.
