Before the incident
Member was placing head dies on the forming table. He went to seat head die with a hammer and it slid off top striking members left hand index finger. Member was wearing gloves. Finger was cut open so member was taken to PIC for medical check. Member was recommended to see a hand specialist appt 11 21 24. Hand surgery done.
