Before the incident
Employee was on the high lift cleaning off the slide gate on top of the mixer hopper [REDACTED] 3 and his supervisor was on the mezzanine. Employee was scraping ingredients out of the way for the slide gate to function properly from the ingredients. The supervisor called him over the radio and warned him the system was coming on. Employee stated he did not hear the call from the supervisor and his left index finger got caught fracturing his finger. When the supervisor turned to look at the panel view is when he heard the employee scream. In turn the supervisor instructed the production supervisor to shut the system down. When the supervisor turned to go help the employee he had managed to pull an air line to free his finger.
