What happened
employee was securing with tie wire a wall form he was stepping on a kicker in the back of the wall (kicker is piece of wood) as he was securing a place of tie wire he made pressure with his leg causing the kicker to come loose his leg went inside the wall form hitting his left knee in a 2x4. He continue working after the incident to finish the task and later reported the incident to the superintendent on site.
