The employee was attempting to read a serial number off the drive belt of a fan

While attempting to read a serial number off a fan drive belt the employee had propped the guard covering to the back of the fan in the open position by using a bolt. When the bolt failed the guard fell and struck the employee causing a laceration to the eyebrow area of the right eye requiring four stitches. The employee was weaing the appropriate head and eye protection.
