What happened
The employee was removing a die cam from a die for repair. The employee placed the die cam on a heel block and held it in place with one hand while he turned to look for a place to set it on a nearby table. While he was turned away the die cam slipped off of the surface it was sitting on and struck his left foot. Employee was wearing steel toed boots but the component fell just behind the edge of the steel toe plate.
