What happened
He was holding the Anti-rotation bar with a pry bar to remove the Actuator. He said that the pry bar slipped and struck him in the mouth which chipped two front teeth. The employee was treated on-site and was later sent to a cosmetic dentist to have his 2 teeth repaired.

He was struck in the mouth by a pry bar. Instead of working the pry bar side to side he was using a front to back motion and when the pry bar slipped as he was coming back it struck him on his mouth.
