Before the incident
Setting a circulation pump on a pad.
What happened
While setting a pump on a housekeeping pad employee was positioning a wedge with left hand while lowering pump down onto anchor bolts. The jack came free and pump was wedged on anchor bolt so employee tapped anchor bolt towards pump and pump slammed down on wedge pinching finger between pad and wedge. Wedge had a sharp edge and when employee pulled hand in reaction wedge cut through and removed tip of finger.
