Before the incident
Manually tightening 3 jaw [REDACTED] on [REDACTED]
What happened
Individual was tightening a 3 jaw [REDACTED] manually using a t-handle on the Doosan [REDACTED] vertical [REDACTED] in [REDACTED]. As he was tightening the chuck he felt something in his arm pop. His arm did not bother him until the next day. Pneumatic tooling (pictured) existed in the area but employee was unable to located the proper attachments to use it. The reason why we use that t-handle is because that chuck is meant for smaller sized parts and bonnets. They tighten by hand so they do not over tighten and potentially crush brass or bronze bonnets. Existing SOP states specific use of the pneumatic impact to tighten 3 jaw [REDACTED].
