Before the incident
Walking from parking lot toward the building where his office is located.
What happened
As the IW was walking his left leg went into an irrigation valve box buried into the ground. The cover to the box was found in the bottom of the box. IW does not feel the cover was covering the hole at the time of the incident. His leg was in the hole up to his knee. Left Leg was received cuts and abrasions. First aid was provided by athletic trainer. The abrasions later developed cellulitis and a doctor prescribed antibiotic. IW also had soreness in right hip lower back hip flexor and hamstring.
