Employee was working as a press operator when they noticed a roll jam up. They attempted to un jam the roll through various methods.

When the operator placed the powered pallet jack under the roll to lift it they caught the end of the pallet jack under the frame of the press. This caused the pallet jack to lift at an angle. Once the pallet jack reached a height of around 18 inches the part of the jack that was in contact with the press frame it slipped off forcing the pallet jack to propel away from the frame and towards the employee. The jack then landed on the employee 's foot.
