Roll Form operator was make adjustments to the roll form tooling

Roll forming associate lifted up the safety cage to make adjustments to the roll former. While the associate was adjusting the machine the strut holding up the safety cage gave way causing the safety cage to fall and strike the associate on the back of the head neck.
