Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a) On or about October 24, 1985, employees were exposed to the hazard of being struck by fragments of a ruptured neutralizer drum or serious burns from teh ensuing fire due to the over pressurization of such drum (Alky Unit #2). One feasible and acceptable method, among others, to correct this hazard is to provide the neutralizer drum or closed relief system with a pressure sensing device with transmitting means to the control room and capability to activate an alarm to alert operatorsof pressure build-up.
