Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to an amputation hazard:
a) Two employees in the Shearing Area feed and cut sheet
metal on a 10 foot Cincinnati Shear (Serial #34134). The
machine foot treadle operating control was unguarded and
only one employee actuates the shear, thus exposing
employees to injury if the treadle was accidently or
inadvertently activated, May 26, 1988.
b) Employees in the Press Brake Area form sheet metal on a
Cincinnati Press Brak (Company Number 9). The machine
foot treadle operating control was unguarded thus exposing
employees to injury if the treadle was accidently or
inadvertently activated, May 26, 1988.
Among other methods, feasible and acceptable methods to abate the
hazards include:
1) Installation of additional machine controls designed so that
all operators must simultaneously actuate the starting mechanism
from their locations.
2) Installation of a foot treadle guard.
