Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Press Brake Area - The light curtain installed on the press brake had sections blanked that created an ineffective area of protection. Employees were not trained to ensure the light curtain was installed and operated to guard employees against contact with the point of operation, on or about 6/22/2011.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free free recognized hazards that were causing or likely to cause death or serious physical harm to an employee in that employees were exposed to amputation injuries at the point of operation of the Cincinati 4 series Mechanical Press Brake: a) Press Brake Area - The light curtain installed on the press brake had sections blanked that created an ineffective area of protection. Employees were not trained to ensure the light curtain was installed and operated to guard employees against contach with the point of operation, on or about 6/22/11. ABATEMENT NOTE: Among other methods, feasible and acceptable means of abatement include but area not limited to complying with the American National Standards B11.3-2002, paragraph 4.2.6 which requires that users are responsible for ensuring that proper installation, adjustment, and operation of the press brake and press brake production system. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
