Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were from from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls and or contact with overhead obstructions: a) Building 3-56 Overhead bridge crane number 554404 - the trolley of an overhead bridge crane with the trolley running on top of the bridge girders was used as a conveyance for employees to reach and replace lamps on ceiling mounted lighting fixtures. On the trolley a 3/4 inch plywood work platform was constructed. No guardrails were provided around the perimter of the platform. The vertical clearance between teh platform and overhead trusses and the horizontal cross bracing was approximately 40 inches. The employees was exposed to being brushed off the work platform by low structural members and/or accidently falling approximately 40 feet to the floor below. Other hazardous conditions noted were as follows: 1) Ceiling fans were so located across the ceiling area that employees are likely to come into accidental contact with rotating fan blades; 2) The operator in the crane cab controlling the repositioning of the trolley did not have an unobstructed view of the employees riding on the trolley work platform; and 3) A safe means was not provided for the employee to go from the bridge footwalk to the trolley work platform. Observed 3/16/88 Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide means to reach and replace lamps such that the employees can be fully protected from the hazardous conditions as mentioned above.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from an unguarded runway or walkway: a) Building 3-57 East Bay North End, Overhead Bridge Crane Number 554421 where two cranes operate on the same runway, one crane is used to board the other. The Northend Crane number 554421 is boarded by running the Southend Crane number 554422 to reach the Northend Crane. At this location the operator crosses the bridge of the Northend Crane and exits by way of the bridge end truck onto the bridge runway. Next, the operator by using the wheel hub cover climbs the bridge end truck of the Southend Crane and onto the bridge to cross over to the operators cab. No grab rails, steps, or other means is provided to safetly reach the boarding platform from the runway. When either boarding or going from one crane to the other, no guarded passageway or walkway or fall protection is provided and the operator is exposed to falling approximately 35 feet to the floor below. Observed 3/17/88. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to provide means to access the crane such that the employees can be fully protected from the hazard of falling.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to in that no training or an demonstration was provided for employees assigned to work from an elevated workstation in the proper use of a controlled decent devise ("Rescumatic") for departure from the cranecab under emergency conditions, thus exposed employees to a fall hazard: a) Building 3-56, 3-57 and 3-60 - all overhead cab operated cranes where the controlled decent device is installed. Observed 3/17/88 Abatement Note: A feasible means of abatement among others is to provide training or an demonstration in such a way as not to create an unnecessary hazard for all employees that work in elevated areas and that may require use of the controlled decent device for emergency egress.
