YARD:  ON OR ABOUT 6/4/93, A POST RACK HOLDING GALVANIZED METAL
BEAMS WAS BEING TRANSFERRED FROM OVERHEAD MONORAIL CRANES TO A
FORK TRUCK.  THE LOAD FELL OF DURING TRANSFRE.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCPTABLE ABATEMENT
METHOD TO CORRECT THE PROBLEM IS TO:
1.   PROVIDE OPERATORS ADEQUATE IN-DEPTH TRAINING IN HOW TO USE THE
CRANES AND HOW TO SAFELY DO THE UNHOOKING OPERATION.
2.   REDESIGN THE PENDANT CONTROL CABLES AND THE CABLE TAKEUP
REELING SYTEMS FOR THE CRANES SO THAT THE OPERATORS CAN
MAINTAIN A SAFE DISTANCE FROM THE LOADS AND FROM THE FORK
TRUCK DURING THE UNHOOKING AND THE TRANSFER OPERATION.
3.   INSTITUTE ADMINISTRATIVE CONTROLS TO ENSURE THAT RACKS AND
PARTS BEING LIFTED ARE WITHIN THE CAPACITIES OF THE CRANES.  THE
LOADS THAT WERE LIFTED EXCEEDED THE CRANE CAPACITIES.
4.   REDESIGN POST RACKS SO THAT THEY POSITIVELY HOLD THE LOADS AND
CAN BE SECURELY ATTACHED TO THE FORK TRUCK.
