Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries. a) On or about 09/26/88 and at times prior to, employees were exposed to serious injury from collapse of the alignment rack. The Benwil alignment rack serial number 1655 would not lock in place on both sides when elevated. One feasible method of abatement, among others, would be to limit the elevation of the rack to two feet or less until permanent repairs can be made. b) On or about 09/26/88 and at time prior to employee(s) working in bay #3 were exposed to crushing injuries druing the unexpected raising and lowering of the hydraulic automotive lift without warning or actuation of the control valve.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to cutting injuries. a) On or about 09/26/88 and at times prior to, employee(s) were exposed to falling panes of glass, approximately 42 inches by 21 inches that had been secured by duct tape in three of the four service doors. One feasible method of abatement, among others, would be to use anchors and glazing compounds.
