Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employee(s) operating the HPM straight side hydraulic press Serial #48-153 on or about October 6, 1987 were exposed to the hazard of the upper die and slide falling as a result of the bolts of the clamp ring working loose or stripping out. One feasible and useful method of abatement is to establish and follow a program of thorough periodic of Hydraulic presses to ensure that all its parts, auxilliary equipment and safeguards are in a safe operating condition and adjustment.
