Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of falling down the hoistway shaft when the
elevator car was not in the landing zone:
a)  Freight elevator, first and second floor landings:
Provisions had not been made to prevent the hoistway doors
from opening while the elevator car was not at the landings,
on or about January 23, 1992.
One feasible and acceptable abatement method, among others
to correct this hazard, is to provide the hoistway doors
with the type of interlocks that prevents the elevator car
from moving when the doors are open, and so latched that
they canot be opened from the landing side unless the
elevator car is within the landing level, so as to conform
with ANSI/ASME A17.3-1986 Part II, Section 2.7 as referred
by Part IV, Section 4.1.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to the hazard of being struck/crushed by a malfunctioning
elevator as a result of not conducting the required periodic and
maintenance inspections and the required tests, so as to keep the
equipment in a safe operating condition:
a)  Freight elevator:  Documentation was not available to
indicate that these inspections and tests were being conducted
to meet the requirements for hydraulic elevators, on or about
January 23, 1992.
One feasible and acceptable abatement method, among others
to correct this hazard, is to follow the requirements for
hydraulic elevators prescribed by ANSI/ASME A17.1-1987,
Section 901, Rules 901.2 through 901.3d.
