Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to the employee in that: Employee(s) were exposed to the hazard of being caught and or crushed between the elevator and shaftway walls or sills. Gate(s) were not provided on the entrance side(s) of the elevator car. Among other methods one feasible and acceptable abatement method to correct this hazard is to: 1) Provide car doors or gates at each entrance equipped with a car door or gate electric contact. Reference Publication ANSI/ASME A 17.3-1986, Section 3.4.2 BOCA Sect. 2105.9.5.
