Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling from elevation when using a
fiberglass ladder:
(a)An eight foot Werner fiberglass ladder (P/N 49336-81) having a
defective frame and leg
was being utilized in everyday maintenance activities including on 3/14/06
when an
employee fell off the ladder while changing a light bulb in the clubhouse
restaurant
resulting in a serious injury to the maintenance employee.  Maintenance
employees have
not received ladder safety training on their proper use, care and
inspection.
1.Provide all employees that use ladders with appropriate ladder training
in correct
ladder use.
2.Perform inspection and maintenance of ladders, and remove ladders from
service
when damaged as described in ANSI A14.5, Portable reinforced ladders,
