Section 1-201(a) of Executive Order 12196 of February 26, 1980. The head %% of each agency shall furnish to employees places and conditions of employment that are free from recognized hazards that are causing or likely to cause death or serious physical harm in that: Laundry operation employees were subject to the hazards of being caught in, crushing, amputation, and death while working to perform inspections, adjustments, and maintenance on equipment and machinery throughout the laundry due to improper lock-out and tag procedures, namely, each worker is not protected with his own lock in that all the maintenance personnel can access any lock. Abatement of the hazard can be accomplished by supplementing the current lock and tag procedures with individual employee locks which can only be accessed by the individual whose lock is in use.
