multiple injuries from falling hazard when employees are elevated in
manbaskets when used with
a powered forklift truck and the manbasket is not secured to the lifting
carriage and/or forks:
a) Mix Building Complex  --- Two workers were working from an unsecured
manbasket which
was elevated 23ft by a GEHL forklift. The manbasket fell of of the forks
throwing both workers
to the ground below fatally injuring one worker.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to secure the manbasket to the lifting carriage and/or forks.
