General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of molten metal eruption/explosions: a) In the Furnace Dept. at No. #3 furnace, the employer allowed wet aluminum ingots and wet scrap bales to be placed hazardously on the ledge at the furnace opening to dry where these materials (wet ingots and wet scrap metal bales) may topple over and have in the past come in contact with the molten metal inside the furnace causing violent explosive eruptions and molten metal splashes. Because the employer allowed and required employees to be exposed to this unsafe condition employees were exposed to serious injuries and death. Among other methods of feasible and acceptable methods to correct this hazard is for the employer to: 1) Immediately discontinue requiring wet aluminum ingots and wet scrap metal bales from being placed hazardously on the ledge of the furnace opening. 2) Immediately discontinue storage of material, aluminum ingots and metal scrap bales outside. 3) Immediately provide employer suprvisor to assure that any and all metal such as aluminum ingots and scrap metal bales are dry prior to being put into the furnace.
