Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a potential amputation hazard or other injury:
(a) For employees operating the freight elevator and exposed to a
potential for injury
created by the lack of an emergency stop switch on the car and other
control panels on
the main floor and basement level.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to follow the requirements set forth in ANSI/ASME A17.1-1984
rule 210.2(e):
Emergency Stop Switch: An emergency stop switch shall be provided in the
car and
located in or adjacent to each car operating panel.  When opened, this
switch shall cause
the electric power to be removed from the elevator drive machine motor and
brake.
