Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer
did not furnish employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or serious
physical
harm to employees in that employees were exposed to an unsafe work practice
caused by the lack of precision in machine control:
FRORIEP VERTICAL TURRET LATHE:  TWO EMPLOYEES WERE
INJURED IN SEPARATE ACCIDENTS THAT INVOLVED THE LACK OF
CONTROL OF THE HEAD (TOOL) DURING POSITIONING.  THE TOOL
TRAVELED TOO FAR, STRIKING ROTATING PARTS AND CUSING THE
PART(S) TO BE THROWN AT THE OPERATORS.  ALTHOUGH THE
COMPANY INVESTIGATED THE FIRST ACCIDENT AND DEVELOPED A
COMPANY POLICY REGARDING THE OPERATION OF THIS LATHE, THE
POLICY WAS NOT EFFECTIVELY IMPLEMENTED AND EMPLOYEES
WERE NOT EFFECTIVELY TRAINED.
ONE FEASIBLE AND USEFUL METHOD TO CORRECT THE HAZARD,
AMONG OTHERS, IS TO:
1.REVIEW AMERICAN NATIONAL STANDARDS INSTITUTE (ANSI)
STANDARD B11.6, SAFETY REQUIREMENTS FOR THE
CONSTRUCTION, CARE AND USE OF LATHES, AND ANY OTHER
RELEVANT DOCUMENTS, TO DEVELOP A PROGRAM TO PROVIDE
ADEQUATE SAFEGUARDING AND TRAINING TO LATHE
OPERATORS.
2.IMPLEMENT EFFECTIVE ENGINEERING AND/OR
ADMINISTRATIVE CONTROLS TO REDUCE/ELIMINATE EXPOSURE
TO THE FRORIEP VERTICAL TURRET LATHE BY THE LATHE
OPERATORS.
