Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards of underhung crane failure: a) Harrington Overhead Underhung Crane - Shear Area of plant had numerous safety deficencies such as: 1) Hoist panel needs to have a flat cable connector plug installed. 2) The weld on the lower block nut is broken and needs to be rewelded. 3) The upper sheave shaft is too short for the application and needs to be replaced with the proper length shaft. 4) The cast end trolley stop pad is missing and needs to be replaced. 5) The off button on the pendant station does not function and needs to be either repaired or replaced. 6) The drive wheels opposite the main collector need to have the bearings replaced. 7) The drive wheels on the main collector side are digging into the rail. This may be caused by improper crane span. 8) A door track hanger is missing on the festoon system and needs to be replaced. 9) The door track hits an angle support for a shelf at the north end of the building. This angle support should be relocated if possible. 10) There is excessive wiring hanging from the main collectors. 11) The crane capacity sign is incorrectly marked on one side. Among other methods, feasible and acceptable abatement methods to correct this hazard is to: 1) Correct all safety deficiencies. 2) Conduct frequent and regular inspections of the crane as referenced in ANSI (American National Standard Institute) requirement B30.11.2.1-1979 Monorail Systems and Underhung Cranes.
