were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing injuries from unintended ram movement as the press
was modified by employer and was done so in such a manner that the
modifications were not in compliance with set safety standards for such
presses:
The alleged violations below have been grouped because they involve
similar or related hazards that may increase the potential for injury
resulting from an accident.
a) Press Area - Employer modified the control circuitry
of the Baldwin 200 ton press in such a way that the
resulting operation of the press was not in accordance with
safety requirements outlined in Section 5 of American
National Standards Institute (ANSI) B11.16 Metal Powder
Compacting Presses - Safety Requiremenets for Construction,
Care, and Use.
1. The two-hand control safeguarding device
was by-passed.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing injuries from unintended ram movement due to
failure in air value or air supply:
a) Press Area - No air pressure switch was installed on
the incoming air supply for the clutch/brake, nor were tandem
or dual air valves utilized to prevent increase in stopping
time due to failure in the operating valve mechanism and
thus exposing employees to injury where employees use a
hands-in-die operation to feed powder and remove green
part from the Baldwin 200 ton press. Press las used on
or about February 21, 1992.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is to:
1. Install air pressure switch on incoming air supply line.
2. Install tarden or dual air valves on air clutch control
system.

were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to crushing injuries from unexpected ram movement while
operating the press in the "inch" mode:
a) Press Area - Employees "inched" the Baldwin 200 ton
press by using a single button control on the control panel.
This control was so located that the operator could easily
access the point of operation of the press. Press last
used on or about February 21, 1992.
Among other methods, one feasible and acceptable abatement method
to control this hazard is to design the "inch" operating means to
prevent exposure of the operators hand written the point of operation
by:
1. Requiring concurrent use of both hands to actuate the clutch,
or,
2. Being a single control protected against accidental actuation
and so located that the worker cannot reach into the point of
operation while operating the control.
