Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed to:
a)  On or about June 12, 1992 in the hallway outside the north     -
cafeteria, an employee was using the Hi-Jacker manually propelled  e
mobil work platform, serial number DH 69044, to access a HMAC      e
unit and the outriggers were not extended and secured.  Among other    f
methods, one feasible and acceptable abatement method is to train
workers in safe procedures and require the use of the outriggers
at all times.
b)  Shift mechanics and pipefitters using the Hi-Jacker manually
propelled mobile work platform, serial number DH 669044, had not   ad
read or been trained in the safe operation of the equipment by
means of the operator's manual as specified on the Hi-Jacker
label.  Among other methods, one feasible and acceptable
abatement method is to obtain an operating manual and to use the
manual to train workers.
