Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Being struck by a load should the overhead hoist, rail or building structure fail; (a)In the shipping/receiving area; overhead hoist without a load rating capacity for the rail or routine inspections. (b)In the specialty department by the glass glazing area; overhead hoist without a load rating capacity for the rail or routine inspection. Glass panels lifted into position with the hoist and a glass vacuum lifter. (c)60 Series Patio Doors in building #5; overhead hoist without a load rating capacity for the rail, no routine inspections and the "U" bolts on the wire rope (below the hook) were installed incorrectly. Among other methods, feasible and acceptable methods to correct these hazards are: 1.Determine the load rating of the supporting structure, ensuring that it is at least equal to that of the hoists, or provide a structure with such a rating, as per ANSI B30.16-1.3.2. 2.Perform frequent inspections at daily to monthly intervals and periodic inspections at monthly to yearly intervals as per ANSI B30.16-2.1., 2.1.2., 2.1.3., or the owner's manual. Written, dated and signed inspection reports should be maintained at time intervals specified in ANSI B30.16-2.1.1(b)(2). 3.Insure that below-the-hook lifting devices (vacuum lifting devices) are designed, installed and maintained in proper condition with trained operators. Perform frequent inspections at daily to monthly intervals and periodic inspections at monthly to yearly intervals per ANSI B30.20-2.1., 2.2., 2.3., and 2.4. Records of periodic inspections shall be maintained, or the device marked to indicate the inspection was performed per ANSI B30.20.ed

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of accidental activation: (a)screening dept. in building #2; screen cutting saw had a metal bar covering the activation button/switch which increased the size and sensitivity of the button which could result in an accidental activation. (b)door extrusion in building #6; the foot pedal for the Sampson Mitre saw was not mounted or guarded from accidental activation. Among other methods, feasible and acceptable methods to correct these hazards are: 1.Provide protection for activation means such as but not limited to: two hand controls, additional point of operation guarding, treadle protection, mounting, and/or increase distance of the activation means from the point of operation.
