employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by the load should the
below-the-hook device fail.
a)IN THE WORKPLACE, Gray-Syracuse, Chittennago NY, on or about 11-04-
97, the following below the hook, man made lifting devices, had no rated
load
markings, no operational test performed and no initial, frequent, or
periodic
inspections performed.
1)Foundry, pour area, overhead hoist with an under hook device for
ladles.
2)Cut and Clean, Caustic area, P & H Crane with under hook device
attached.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1)Prior to initial use, all new altered, modified or repaired lifting
devices shall be inspected by a designated person and an operational
test performed to verify compliance.
2)Mark the rated load of the lifting device on the main structure where
it is visible.
3)Conduct frequent/ periodic inspections and perform the required
maintenance following these inspections in accordance with ANSI
B30.16-1987 Chapter 16-2.
