Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that on or about 6/15/92,
employees operating or riding freight elevator were exposed to the hazard
of being stranded in a non-functioning elevator, due to malfunction;
power failure, fire and had no emergency communication system available
for use in the elevator.  One feasible and acceptable abatement among
others to correct this system as stipulated in ASME/ANSI A17.3 Section
3.11.1.
a)  66 Furman Street, Brooklyn, N.Y. - Elevator car #2738 and
2739 on West side of building was not provided with an
emergency signal system.
The National Cold Storage,Inc  was previously cited for a violation of
this Occupational Safety and Health Standard or its equivalent
standard, 5(a)(1), which was contained in OSHA Inspection Number
109895979, Citation Number 1, Item Number 1, issued on
8/17/90 with Final Order Date of 9/6/90.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that on or about
6/15/92, employees operating or riding freight elevator were exposed
to falling into freight elevator shaft which could cause serious
injuries or death.  The electric freight elevator mechanical interlock
and contact on first and fourth floor had a piece of wood wedbed into
the mechanical lock/contact device which with hoistway doors in the
open position.
ABATEMENT NOTE:
A feasible and acceptable abatement method among others
to correct the hazard is to follow ASME/LANSI A17-1 1989
Section 111.12, not permit to install a device which could
rended hoistway mechanical locks and electric contact
inoperative, remove bypass material (wood) and instruct
employees or danger of bypassing interlock.
The National Cold Storage,Inc  was previously cited for a violation of

to the hazard of being crushed between moving elevator car and all
of the elevator shaft due to car not having a door or gate with
interlock, to be closed before elevator could be oeprated.

were not provided with doors/gates with interlocks.
ABATEMENT NOTE:
One feasible and acceptable abatement method among others
to correct this hazard is to take freight elevator cut off
service, install gates/doors as stipulated in ASME/ANSI
A17.3 1986 Section 3.47 ASME/ANSI 17.1 1989 Section 111.5
(o), 111.5(c)(1), 111.5(c)(2), 204.4(a) and (b)
