or serious physical harm to employees in that: On or about 4/26/90 -
Employees were exposed to the hazard of being stranded in a non-
functioning elevator. There was no emergency communication system
available for use in the elevators:
Location: Elevators 12, 13, 14, & 15 in Building #24.
One feasible and acceptable abatement method, among others, to correct
this hazard is: 1) Repair or install the appropriate emergency signal
system as stipulatd in ASME/ANSI A17..1-1965 Rule 211.1 and 321.11.

or serious physical harm to employees in that: On or about 4/26/90:
Employees were exposed to the hazard of being crushed between the moving
elevator car and the walls of the elevator shaft due to the elevators
not having a door or gate which would have to be closed before the
elevators could be operated:
Location: Freight elevators 12, 13, 14 & 15.
One feasible and acceptable abatement method among others to correct
this hazard is: 1) Take the freight elevator out of service until a gate
is installed as stipulated by ANSI 17.1-1965 files 111.5(b), 111.5(c)(2)
111.5(c)(1), 204.4(a), 204.4(b), 204.6(a).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that: Employees were exposed to
the recognized hazard of being crushed by, or falling off, elevators or
floor landings into empty shafts:
a) On or about 4/26/90 freight elevators #12, and NRS. 5, 7, 13, 14 and
15 had malfunctioning or bypassed interlock systems (using tape,
sticks, cut wires, etc.). This condition allowed the operation of
the elevator doors, and while the hinged platform sill was not in
its fully retracted position.
One feasible and acceptable abatement method, among others, to correct
this hazard is:
1) Remove Bypass material (tape, sticks, etc.), replace cut wires and
close the elevator doors on all floors until the safety interlock
engages before operating the elevator.
