The hazards of fire and explosion on September 26, 1989 while
placing tubing into the Milam #4 gas well located off the west
end of Franklin County Road #47, southwest of Ozark, Arkansas,
when the 75% methanol, 25% water solution unloaded from the well.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to utilize blow-out prevention equipment when
bringing wells into production.

The hazards of fire and explosion from approximately September 1-26,
1989 when working in the derrick of Drilling Rig #17 with no safe
means of escape, while drilling the well and placing tubing into the
Milam #4 gas well located off the west end of Franklin County Road
#47, southwest of Ozark, Arkansas.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install an auxiliary means of escape such as a
specially rigged and securely anchored escape line so as to provide a
ready and convenient means of escape from the derrickman's working
platform.
