Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to the hazard of being caught in a pinch point.
On or about 01-28-2000, where employees were required to operate the
packer blade equipment
on a White/GMC side loading residential waste truck:
a) The employer failed to ensure that the operating controls of the packer
blade were
conspicuously labeled as to their function. Among others, one feasible
method to abate the
hazard is to label all operational controls as to their function in
accordance with American
National Standards Institute (ANSI) standard Z245.1-1992, paragraph
7.1.4.1.
b) The employer failed to ensure sustained manual pressure controls for
the
packer blade were
installed and maintained. One feasible abatement method, among others,
would be for the
employer to install constant pressure switches for the packer blade
operation in accordance with
ANSI Z245.1-1992, paragraph 7.1.4.2.
c) The employer did not ensure that electrically controlled components
operated by push buttons
had the push buttons recessed or located so as to prevent unintentional
activation in accordance
with ANSI Z245.1-1992 paragraph 7.1.4.3, and the vehicle manufacturer's
maintenance and
operating manual. One feasible abatement method, among others, is to
install recessed switches
to operate the packer blade equipment.
Abatement documentation must be submitted for this item.
