Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about June 2, 1992, the four (4) Werner Task-Master Stage Platforms located at the number 3 superheat furnace at the Tennessee Valley Authority's Gallatin Steam Plant were not equipped with nonconductive building face rollers, exposing employees to the hazard of falls. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install nonconductive building face rollers on all stage platform or two point suspension scaffolds.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about June 2, 1992, the four (4) Werner Task-Master Stage Platforms located at the number 3 superheat furnace at the Tennessee Valley Authority's Gallatin Steam Plant were not grounded to the building structure and/or the tubes being cut or welded, exposing the employees to the hazard of falls. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ground the scaffold to the building structure at the point where the welding and cutting are performed.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about March 6, 1992, the wire rope hoist lines and other metal parts of the four (4) Werner Task-Master Stage Platforms located at the number 3 superheat furnace at the Tennessee Valley Authority's Gallatin Steam Plant were not insulated to prevent contact with the building structure, causing wire rope hoist lines to arc into, exposing employees to the hazard of falls. Among other methods, one feasible and acceptable abatement method to correct this hazard is to insulate all metal parts of the stage platform and rigging that may come in contact with the building surfaces.
