Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) A rotating blade on the componamatic saw during maintenance, repairs
and adjustments. The exposure is, in part, the result of frequent
failure of the power brake upon deactivation of the saw allowing the
blade to rotate freely. The exposed employee either adjusts the
rotating blade, or prior to making adjustments, stops the blade by
applying pressure to the blade with a piece of wood. The employee is
also exposed to the potential hazard of inadvertent activation of the
saw during maintenance, repairs, and adjustments.
b) Rotating blades on the economatic radial arm saw during maintenance,
repairs and adjustments. The saw was not equipped with a braking
device allowing the blades to rotate freely upon deactivation.
Employees adjusting one blade, were working in close proximity to
the other rotating blade. Employees are also exposed to the potential
hazard of inadvertent activationof the saw during maintenance,
repairs, and adjustments.
c) Among other methods, some feasible and acceptable abatement methods
to correct these hazards are:
1) Repair power brake on componamatic saw.
2) Fabricate and install a braking system on the economatic saw.
3) Develop and implement a lockout/tagout program, as stipulated by ANSI
Z244.1-1982, Section 4 and 5, American National Standard of Minimum
Safety Requirments For Lockout/Tagout of Energy Sources For Lockout/
Tagout of Energy Sources For Personnel Protection, to ensure that
equipment is at a zero energy and movement state prior to maintenance
repairs, and adjustments.
